How to report a concern

 

Arizona State University is committed to maintaining a safe, respectful and inclusive educational environment where students can learn, work and participate in university programs free from mistreatment, including discrimination or harassment. If you have a concern about conduct or behavior that may be affecting the learning environment or student safety, please use this form so we can connect those involved with appropriate resources and, when needed, review the matter for possible response under applicable university processes.

Conduct concerns

Conduct concerns that may impact the learning or work environment or safety — independent of any allegation based on protected status — can be addressed by submitting this form.

Examples may include:

  • Public humiliation.
  • Threats of physical harm.
  • Physical harm (e.g., hit, slapped, kicked).
  • Being required to perform personal services (e.g., shopping, babysitting).

Discrimination and harassment

ASU prohibits discrimination, harassment or retaliation based on protected status, including race, color, religion, sex (including Title IX sexual harassment, sexual orientation and gender identity), national origin, age, disability, veteran status and genetic information.

Examples may include:

  • Subjected to unwanted sexual advances.
  • Asked to exchange sexual favors for grades or other rewards.
  • Denied opportunities for training or rewards based on a protected status.
  • Subjected to offensive remarks/names based on a protected status.
  • Received lower evaluations or grades solely because of a protected status rather than performance.

  • If expecting anonymity with any reporting form, do not include any identifiable information including your name, email or student ID number. Please know that anonymous reports can limit the ability to gather information regarding your concern.
  • ASU also has an ASU Ethics Hotline that allows reporting anonymously.

Mandatory reporting: All ASU employees are mandatory reporters for allegations of sexual harassment, and supervisors are mandatory reporters for allegations of any protected status harassment. Information received regarding such allegations will be shared with the appropriate university office for review and response.
